Meps medical form question....?

i was on stress meds when i was about 14 for.. four about 18 months.... i am now 19... i was wondering if i put done the fact i have unknown reasoning for borderline blood pressure levels... would meps see that on the medical records?? or would i be the one brining the records to meps?
